Medication Log (to be included completed and returned with the New Patient Packet)
Please fill out the required forms and bring them with you on the day of your appointment. Our neurologists require that you bring your medical records from your referring physician, along with any of the following items that your physician may have concerning your medical history: blood test, x-ray reports/films, CT scan reports/films, MRI reports/films, and the physician's notes. Also, it is very important that you bring your insurance card(s) and a picture ID on the day of your appointment.
